This is the information that you need to provide to Phoenix ASA to receive a 2008 Certificate of Insurance / Additional Insured
Here is what you need to provide to receive a certificate of insurance /Additionally Insured:
NOTE: You need to complete your 2008 Phoenix ASA Registration before requesting this.
1.) Name of: City / School / Company etc that wants certificate.
2.) Specific name of the location. example: Shea Middle School / Rose Mofford / Jewish Community Center, ASU etc.
3.) Complete Address: Street with number / City / State / Zip Code
4.) Contact person: if you need that on the form.
5.) I will e-mail you a copy.
